NOVAMag – a New Generation of Biomaterial

Guided bone-regeneration techniques use either resorbable or nonresorbable membrane. Ideal membrane material should be biocompatible with tissue integration, be able to create and maintain space, be occlusive with selective permeability, and have good handling properties.

In this workshop, you will be able to get to know and practice two soft-tissue management techniques. Indications as well as the advantages and disadvantages of each technique will be discussed.

Prof. Dragana will lead the bone tissue management and Dr. Luka will lead the soft tissue management.

NOVAMag – a new generation of biomaterial

1. Guided bone-regeneration techniques use either resorbable or nonresorbable membrane.

Ideal membrane material should be biocompatible with tissue integration, be able to create and maintain space, be occlusive with selective permeability, and have good handling properties. Commercially available nonresorbable membranes are e-PTFE, d-PTFE, and titanium mesh. Resorbable membranes are available as natural and synthetic. In the world of recent dental implantology times are beginning to change, and the demand for synthetic options is on the rise.  The use of absorbable membranes is extremely widespread due to their advantages, but in clinical situations of significant tissue loss, the use of non-absorbable membranes is often still preferred. Barrier membranes made of non-absorbable material are characterized by mechanical stability over time so that the by-products of degradation of the basic materials are not a cause for concern, and the period of fixation in place is easy to control. The major disadvantage of these membranes is the need for a second surgical time, removal from the site, after the completion of tissue neo-formation. The most common complication of nonresorbable membranes is exposure. NovaMag system, the new concept of resorbable magnesium (metal) membrane and biodegradable magnesium (metal) fixation screws makes a unique combination between conventional resorbable and non-resorbable membranes, with all the advantages of both, and with reduced complications. as the only metal resorptive membrane so far, it is a great novelty in the world of augmentative techniques.

 

2. Soft tissue managment after GBR procedure (collagen matrix and free gingival graft techniques) – dr. Luka Marković, Specialist in Periodontology

After vertical and horizontal GBR procedures in most of cases there is a lack of soft tissue, vestibule loss and keratinized gingiva loss. Lots of studies suggest us that minimally 2 mm of keratinized gingiva is required for long term stability around implants. Problem of the free gingival graft (FGG) is a shrinkage, color and morbitity for the patient. Collagen matrices have showed great advantage in a long term stability and better esthetics. Technique of combination of FGG as a autogenous tissue and collagen matrix give us the best combination and it’s very predictable for reduction of shrinkage, matching colors and minimal patient’s morbitity for reestablishment of vestibule loss and keratinized gingiva.

Minimally invasive procedures as well as using the right harvesting technique will result in less pain for the patient, fewer complications and an aesthetically-pleasing successful result.

In this workshop you will be able to get to know and practice two soft-tissue management techniques. Indications as well as the advantages and disadvantages of each technique will be discussed.

This course includes additive and respective soft tissue management in the aesthetic and functional zone around teeth and implants

Local, sustainable, organic, recyclable: intraoral donor sites

Abstract:

This lecture explores the diverse range of intraoral donor sites that can be used for autologous tissue harvesting beyond what is typically covered in dental school and training. By utilizing autologous tissue, we can avoid the risk of disease transmission and take advantage of its growth factors. However, the gold standard tissue is often overlooked due to concerns around donor site morbidity. Selecting donor sites that are adjacent to the recipient area is associated with minimal postoperative discomfort. Specifically, the focus is on the utilization of tuberosity bone, translocated ridge segments, and extracted or partially extracted teeth. Additionally, complications that may arise when biological principles are not followed will be reviewed. This lecture’s goal is to expand the knowledge of intraoral donor sites and provide practical guidance for their successful utilization in clinical practice.

Short bio:

Snježana Pohl is an oral surgeon and periodontist with dual doctorates in human and dental medicine. She is an oral surgeon in Rident, a private clinic in Croatia. In addition to her clinical practice, she holds a faculty appointment as an Assistant Professor at the University of Rijeka’s School of Dental Medicine, Department of Oral Medicine and Periodontology. Snježana Pohl is the author of numerous scientific papers. Her work focuses on the importance of minimally invasive techniques that preserve tissues. She has particular expertise in Partial Extraction Therapies, Autogenous Tissue Utilization, Osseodensification, and other innovative approaches to implant surgery. She is the author of the app for implant surgery in the aesthetic area: Decision matters.

Advanced Full Arch and All on X Hydrophilic Implant Treatment

Abstract:

Implant dentistry is growing and with its growth, it is bringing about revolutionary changes to treatment plans and protocols. For what was once considered to be a far-fetched treatment option is now more common among the patients. The comprehensive course “All for one and one for all” is primarily designed for individuals who have adequate experience in Implantology. A fine background on Implantology is vital for this course, as the focus here is on minimizing the complications encountered during implant supported prosthetic treatment in completely edentulous patients. The All-on-4 and All-on- 6 concept advocates distal tilting of implants in edentulous arches, which enables placement of longer implants and improved support with short cantilever arm. Inter implant distance is improved thus promoting improved anchorage into the bone while providing immediate full arch rehabilitation with lesser number of implants. This course gives you an introduction to effective treatment alternatives that have been proved to have higher clinical success rate. This course bridges that gap between the surgical and prosthetic treatment procedures enabling a structured treatment plan which produces immediate aesthetic results thus increasing its popularity among patients. This course manifests the idea for a better, conservative, comprehensive and cost-effective treatment.

Prof. Dott. Angelo Cardarelli Graduated Cum Laude in Dentistry and Dental Prosthetics at the University of Aquila in 2007. Specializing in Oral Surgery at the Sapienza University of Rome in 2010. He is an Adjunct Professor and tutor at the San Raffaele University of Milan. Scientific Advisor to the Dental Clinic at the Department of IRCCS San Raffaele in Milan (Dean Prof. E. Gherlone). Visiting professor of the International University of Agadir (Universiapolis). Member of Global Scientific Dental Alliance in Dubai. Works in Milan, Isernia, and Dubai, where he practices Implanto-prothesis and Oral Surgery. Author of many publications, Speaker in International Congresses on Implantology and Oral Surgery. Member of the International Extraction Academy of California. Author of the Book: “Piezoelectric Surgery of Impacted Teeth“ EDRA Publisher 2020.

Interdisciplinary Management of Aesthetic & Restorative Dilemmas

Abstract:

The Ortho-Perio-Restorative interface plays a critical role in the management of worn and broken down dentition, patients with congenitally missing and malformed teeth and patients with severe alveolar defects or advanced periodontal disease involving teeth in the aesthetic zone. This presentation will outline the interdisciplinary team approach to managing our cases predictably in our practice. The aesthetic rehabilitation of the periodontally and structurally compromised dentition in the aesthetic zone remains a challenge despite the numerous advances in the treatment of periodontitis, regenerative and dental implant therapies. The management of some cases requires some ‘out-of-the-box’ thinking.

Short bio:

Dr Tidu Mankoo qualified with a BDS from the University of Bristol in 1981 and has a renowned Private & Referral Practice in Windsor, UK, treating Implant, Restorative and Aesthetic cases, particularly complex cases. He has built a reputation for exceptional dentistry and carries out both the surgical and prosthetic aspects of his cases. He is an active member of the European Academy of Esthetic Dentistry, the Association of Dental Implantology (UK), the European Association of Osseointegration (EAO), Fellow of the International College of Dentists (FICD), and a fellow of the International Congress of Oral Implantologists. He is a Visiting Professor at the University of Catalunya, Barcelona, Spain. He was the President of the European Academy of Esthetic Dentistry 2009-2010 and a founder and Past President of the British Academy of Aesthetic Dentistry and serves on the Editorial Board of the International Journal of Esthetic Dentistry He is a respected teacher with numerous publications. He is a sought-after international speaker particularly in the field of Dental Implants, Aesthetic Dentistry and management of complex cases. He lectures and gives courses regularly all over the world.

Abstract:

Advanced Immediate implantation and immediate loading in the aesthetic zone
 Systematic planning prepares you for a case requiring) an immediate implant with immediate loading. The main advantages of immediate implant placement are that they save time and there are fewer patient visits. During implant surgery, the dentist has to make a lot of decisions that are based on scientific knowledge and his own experience and personal skills. There are numerous indications for tooth replacement with an immediate implant when an adequate amount of bone and soft tissue are available to support it. Teeth need to be removed atraumatically to preserve the maximum amount of bone before immediate implant placement. The harvesting of CTG from the palate for soft tissue management can improve the result in the aesthetic zone. This lecture will include hints and tips on all aspects of immediate implantation, hard and soft tissue management with CTGs, which you can take back to your practice. Indications as well as the advantages and disadvantages of technique will be discussed.

Abstract:

Biomaterials are natural or synthetic substances and help to repair, augment, or replace any tissue or organ of the body. Various biomaterials can be used to augment bone for future implant placement. Bone grafts are the type of biomaterials that are used to substitute bone defects and recover atrophic bone regions. Each graft procedure has advantages and disadvantages in each clinical application and needs to choose the graft material with a high success rate and less morbidity. Today, there are various advancements in biomaterials and methods for bone augmentation. The use of different scaffold material types, stem cells, and growth factors shows promise in regenerative treatment interventions for alveolar bone defects. No single biomaterial or clinical technique is ideal, and the clinicians need to decide the suitable approach which can provide suitable results with less complication. Each graft procedure has advantages and disadvantages and should use the material with a high success rate and less morbidity. It is important to have a clear and detailed understanding of the fundamentals in regenerative science for successful outcomes in bone grafting.

Osteo-Immunology : A New Paradigm Shift in Implantology and Bone Grafting

Abstract:

Osteoimmunology is the modern science that shows the influence of immune system on bone metabolism but also on all tissue healing phenomena. Indeed, bone healing starts with an immune reaction: inflammation triggered by the release of inflammatory cytokines. These cytokines are responsible for angiogenesis, which activates the formation of bone callus. The behavior of bone cells is dependent on the functioning of the immune system. High level production of oxidants is named oxidative stress, will lead to a failure of the immune system with collateral effects such as infection, bone resorption or fibro-integration. This is the case, for example, in diabetics, smokers, patients allergic to penicillin or in chronic inflammatory diseases. A favorable bone metabolism will necessarily require a reduction of oxidative stress by means of various solutions such as adequate vitamin D supplementation, reduction of LDL cholesterol levels or other adequate immune stimulants. Antibiotic prophylaxis must also be revisited in order to use antibiotics with significant immune-stimulating properties. PRF has also an immune function : decreases the inflammatory level and increases the local anti- oxidants. This new science favored new clinical protocols : the open wound technique. With a powerful immune system there is no fear of infection, It will be easy to demonstrate that peri-implantitis is an immune disease that causes bone loss around the implant, triggered by oxidative bone stress.

Simplified lateral ridge augmentation by f.i.r.s.t. and one-time cortical lamina

Abstract:

The Fibrin Sealant (FS) is a drug of human origin used in general and specialist surgery for many decades. It is validated by over 6,000 papers in the literature. In oral surgery, FS stabilizes and maintains the natural clot for 10-14 days. It has adhesive, sealing and inductive action for hard and soft tissues. FS allows better management of particulated bone graft that, becoming plastic and adhesive, adapts perfectly to defects without shedding of granules. It also protects surgical wounds and stimulates faster soft tissue healing. The F.I.R.S.T., devised by Foti V. in 2016 and published in 2020, is a GBR technique that stands out for its simplicity, predictability and low morbidity. Based on the triad fibrin sealantcollagenated heterologous bone-heterologous cortical lamina, it allows the reconstruction of two/three walls post-extraction sockets using envelope flaps (Lamina Shield). In lateral GBR the F.I.R.S.T. allows to glue cortical laminae for heterologous bone particles containment without pins or screws. The result is a lamina/graft complex strongly adherent to the defects, without micromovements and empty spaces, that is one of most important prerequisites for the neo-angiogenesis and the new bone formation. Finally, the minimally invasive “One-Time Cortical Lamina” technique, published by Foti V. in 2021, where it is achieved a vestibular lateral ridge augmentation to simultaneous implants that does not require surgical re-entry for uncovering.

Decoding the aesthetic zone in implant therapy: When, what and how for optimum aesthetics

Abstract:

Once the decision is made to extract a tooth, clinicians have a decision to make; delayed management – i.e. extract and wait, a staged approach – i.e. graft and wait or immediate implant placement. Immediate implant placement combined with a later or an immediate soft tissue support? Also, the achievement of an aesthetic implant-supported restoration is a constant challenge to the restorative dentist. Due to the circular shape of the implant and its smaller diameter, when compared to the root of a natural tooth, a dilemma inevitably occurs of how to construct an artificial crown that will imitate the natural tooth crown form. The appearance of the surrounding soft tissue the is of major importance, and various techniques have been developed to guide its topography. This presentation will highlight parameters and key factors for the evidence-based decision making in order to choose the technique that facilitates a more aesthetic, functional and predictable result according to the patient expectations.
